A free personal introduction
The songs you know by heart. A morning routine you enjoy. The people and stories that matter to you. Make an All about me profile for someone who is getting to know you, at home or in a care setting.
Eight optional starting points · Your own words · Text download · Browser printing · No account
Your words stay in this page's memory. They are not autosaved: refreshing or closing clears them. Download text or print before leaving.
Fill this in for yourself, or ask the person what they would like others to know. Every field is optional. Keep their wording and leave out anything they would rather keep private.

Choose a comfortable time to fill this in together. Ask what they would like to include and who they would like to have a copy. Use their preferred name and wording; a few lines are enough. Put a date on the profile and update it when their preferences change.
The Alzheimer's Association describes sharing personal history, interests and routines with care providers. These original MemoirWell prompts offer space for that personal introduction. Use your care team's usual forms for medical needs and care instructions.
Yes. You can introduce yourself to a new caregiver, care-home staff member or anyone you choose. Type your own words, skip any field, or print a blank profile to fill in by hand. The form covers personal history and preferences; it does not assess care needs or create a medical care plan.
Download text saves a TXT file. Print or save PDF opens your browser's print dialog; choose Letter or A4 and check the preview. Printing also works directly from the browser menu while you are editing. Longer answers may use more pages. The TXT can be edited in a text editor; this tool cannot reopen it as a filled profile.
They stay in this page's memory. This tool does not upload your entries, use AI or autosave them. Refreshing or closing clears everything you entered. Download or print before leaving. You choose who receives your saved copy.
Yes. Ask the person which parts they want to share, keep their own wording and note who prepared the profile with them. You can leave a question blank or return to it later. Share the saved copy yourself; this tool does not send it to a provider or create a shared online record.
Save your profile first. MemoirWell's separate guided interview can help you develop one remembered scene into a draft chapter to check and edit. Your profile is not transferred automatically.
