Before the plans are fixed
A family reunion survey you can fill in and print.
Start by asking what would help people take part. Keep one household’s preferences about timing, travel, meals and activities together, with room for what is still undecided.

Getting the worksheet ready…
Use the survey before choosing the details
An early survey gives relatives a way to describe what could work. Ask for a date range rather than a promise, leave room for travel limits and invite an activity or memory someone would enjoy sharing. An unanswered question can remain open.
The Cobb Travel & Tourism reunion workbook (PDF, family survey on page 16) asks about dates, place, food, activities and volunteering. The Reunions magazine forms (PDF, family reunion survey on page 2) include timing, travel, spending, activities and help. The questions and layout here are original MemoirWell wording.
A fictional answer
Household: Jo and Rosa. Dates: A Saturday in June could work; the last weekend is uncertain. Travel: Within a day’s drive, with a place to sit indoors. Activities: Bring one old photograph and ask everyone what they remember. Reply instructions: Give this printed sheet to the organizer at Sunday lunch. These people and plans are invented.
Keep preferences separate from confirmations
This survey is for exploring a plan. After the organizer chooses a date and place, use the separate guest list to keep response notes and the next detail to confirm. You arrange how to share and gather the saved surveys; this page does not send invitations or collect answers online.
Before you save the survey
Can I print a blank family reunion survey?
Yes. A blank copy includes all six questions with handwriting lines. You can add a title and reply instructions first. A partly answered copy also keeps every question, so someone can finish it on paper.
Is this an online RSVP form?
No. It is a worksheet for one household’s early preferences. It does not collect responses, send emails, count guests or confirm attendance. Save a blank copy to share or complete your own answers, then arrange delivery yourself.
Where are my answers saved?
Only in this page’s memory. They are not uploaded or autosaved; refreshing or closing clears them. Download text or print before leaving. Saved TXT and PDF files cannot be reopened here as an editable survey.
What happens when I clear an answer?
Clear this answer asks you to confirm. Undo restores its wording until you write new words in that answer or close the page. Answering another question leaves Undo available. Your title, household and reply instructions are unchanged.
How do I print or save a PDF?
Use Review my survey, then Print or save PDF, or the browser’s print command while editing. Choose Letter or A4 and check the preview. Long answers or a long heading can use more pages; all six questions remain in order.
Did a question bring back a family memory?
Save the survey first. MemoirWell’s separate guided interview can help you develop your own memory into an AI-assisted draft chapter to check and edit. Survey answers are not transferred automatically.
