Getting the worksheet ready…
One size and shirt option on each line
A household may need several entries: two adult medium shirts on one line and a youth large on another. Write the size range and style in full so the same letter does not mean two different garments. Confirm the printer’s size chart, color names and artwork instructions before you place an order separately.
The Jotform family reunion T-shirt form description lists shirt size, quantity and customization options. A Family Tree University reunion guide hosted by Family Tree Magazine includes tips about collecting attendees’ sizes and using order forms. This worksheet’s fields, example and layout are original; no publisher template or shirt artwork is reproduced.
Leave an unknown quantity blank
The total adds only whole quantities from 1 to 999 on entries you have filled. It reports blank and unrecognized quantities separately. Matching names or sizes stay on their own lines; the worksheet does not merge them. This is a count of entered shirts, not attendance or a confirmed purchase.
A fictional example
Rowan cousins afternoon. Jo’s household: adult M, green crew neck, quantity 2. Next entry: youth L, green crew neck, quantity 1. Total entered: 3 shirts. Personalization: check the spelling of the family name. Contact reference: address-book entry J. These people and plans are invented.
Before you save the shirt list
Is this a free printable family reunion T-shirt order form?
Yes. Fill up to twenty entries or print the blank form with fifteen handwriting rows. A title without any shirt entries still prints the blank form. A filled copy starts with a table and total, followed by personalization, contact references and notes only where you wrote them. Untouched entries are skipped; original entry numbers remain.
How are quantities counted?
Use whole numbers from 1 to 999 without symbols or separators. Surrounding spaces and leading zeros are accepted. An empty quantity is unknown. Zero, negative numbers, decimals and other formats are kept as entered and marked not included in the total, including in the saved copy. Twenty quantities of 999 total 19,980 shirts. No prices, tax, payments or money totals are calculated.
Will this send an order or collect people’s requests?
No. You gather the information and enter it yourself. This worksheet does not contact a printer or family member, collect replies, take payments, reserve stock or arrange delivery. Confirm sizing, availability, spelling and any order terms with your chosen printer separately.
Are my entries saved or uploaded?
They stay only in this page’s memory, without upload or autosave. Refreshing or closing clears them. Download text or print before leaving. Saved TXT or PDF copies cannot be reopened here as an editable form. Every field you fill appears in the saved copy, so use a contact reference instead of contact details if that is enough.
How do I print or save a PDF?
Choose Review my form, then Print or save PDF, or use the browser’s print command while editing. Choose Letter or A4 and check the preview. Long tables and notes can continue onto later pages. Your wording, entry order and quantity warnings are preserved.
Keep the story behind the gathering too
Save your shirt worksheet first. MemoirWell’s separate guided interview can help you develop your own memory into an AI-assisted draft chapter to check and edit. Shirt entries are not transferred automatically.
